Grade Reports
Grade reports demonstrating a cumulative GPA of 4.00 for their first two semesters of university. The grade report for both semesters should be submitted by the deadline.

Personal Faith Statement
Provide a statement of Faith (Are you a Christian? How long? What has Christ meant to you? Has it made a difference in your life?). Explain in a statement of 200 words or less.

Religious Leader Recommendation
From from a parish priest, pastor or chaplain, must be someone who knows your spiritual journey

Certificates of Leadership
These should show evidence of service done during high school or university.

Faculty recommendation
(To be filled by Dean of Faculty/HOD/Lecturer – in case of a lecturer, he/she must have taught or supervised you)
